Patient details in response. Risk score is calculated per patient based on patient's demographic relative factors and associated diagnosis and prescription drug codes. Multiple diagnosis codes and disease interactions are associated with a patient and one or more HCCs are associated with a diagnosis code. Multiple RXCs and RXC interactions are associated with a patient and one or more HCCs are associated with a diagnosis code. s the sum of the factors of the corresponding metal level.
